This is an awesome lesson in shoulder anatomy. This cord-like middle glenohumeral ligament is better known as a “Buford Complex,” named after my colleague and friend Dr. Don Buford Jr., who originally described this anatomical variant in his 1994 paper in Arthroscopy. This variant of normal capsulolabral anatomy is present in about 1.5% of shoulders. Clinically, it is vital that the surgeon recognizes this as a NORMAL variant! In its presence, there is no anterosuperior labral tissue; the glenoid rims appears “bare” below the ligament. Therefore, if the complex is mistaken for the anterosuperior labrum and “reattached” to the glenoid rim, a severe painful restriction in external rotation and elevation of the shoulder will result. This picture may not seem like a big deal to most people, but it is to Kelly. One year ago she was hit by a drunk driver shattering her left humerus and dislocating both her knees! My trauma colleague Dr. David Wellman worked his magic on her arm, and I took care of her knees. There are four main ligaments in the human knee. I repaired five out of eight of her ligaments primarily with no reconstructions. So, after her ordeal simply being able to squat like this IS a big deal!  Well done Kelly! #savetheacl
