A crisp cupid's bow can be accomplished with the cannula. The vermillion border (VB) and cupid's bow are best approached with a 27g cannula for precision. Entry ports are on each side adjacent to commissures. The depth is very superficial with the cannula visible via tissue blanching with passage. Stabilization is accomplished by pinching the VB between thumb and forefinger of non-injecting hand. The lip may be threaded onto the cannula (see earlier post -- "inchworming") or stretched to stabilze. My approach takes advantage of addressing the same slope direction of the arms of the bow as the cannula path. Starting with side one, I pass the cannula through the VB to the proximal Glogau-Klein (GK) point dropping a microbolus there. On the retrograde I fill the lateral arm of the bow. Then, I redirect the cannula towards the contralateral GK point dropping a microbolus there, then I fill the contralateral medial arm of bow on the retrograde. Now, I switch sides to the other commissure and repeat. I pass the cannula to the proximal GK point dropping a minibolus there filling the lateral arm of bow on the retrograde. I then withdraw and redirect the cannula reaching to the contralateral GK point, minibolus, and fill contralateral medial bow on the retrograde. Filling the GK points from both sides insures proper definition. All 4 arms of filler outlined above meet to complete the cupid's bow. This is all accomplished quickly with practice. Why not just use a needle? Because a cannula avoids multiple needle sticks, bleeding, bruising, and when proficient, it can be less painful.
