You know you’re in the right place when you get home from a long day of craniofacial clinic and can’t wait to explain your slick, new surgical education toy to your one year-old. It would be appropriate to imagine the rest of this post in a barrage of enthusiastic attempts at Sesame Street imitations. . We regularly use 3D printing in both craniofacial and microsurgery, and I wanted to adapt that technology for an educational purpose. A company that performs a lot of our virtual surgical planning helped me design and make this teaching skull. When the craniofacial skeleton doesn’t form or develop properly, we often mobilize segments in very specific ways in order to rearrange, advance, or distract them (slow advancement to allow bone to grow between separated parts). With the foundation re-established, we can then fine tune soft tissue aesthetics or functional movement with a wide variety of synthetic implants, autogenous tissues, and other tissue rearrangements. . The osteotomies (bone cuts) for Lefort I, II, III, monobloc advancements, box osteotomies, and facial bipartition can be difficult to imagine in 3D space from 2D drawings alone. Nothing beats hands-on experience, but these concepts are particularly tough to fully understand, as a trainee, intraoperatively while navigating confined spaces with bulky soft tissues (inside the orbit against the eyeball or inside the cranium against the dura/brain), and retractors and hands in the way, so a model is a perfect happy medium. This skull has full thickness osteotomies marked in a clear cast to allow medical students and residents the ability to inspect the cuts and more easily conceptualize a complex operation in a simple fashion. . When I was learning these techniques, I used several textbooks, and now I refine that knowledge with primary literature.
