bilateral subdural accumulation of air causing compression of the frontal lobes
this pattern resembles the silhouette of a volcano, such as Mount Fuji
it is indicative of tension pneumocephalus, which warrants clinical correlation and emergent neurosurgical evaluation
if tension pneumocephalus is confirmed, management includes emergent decompression (drilling of burr holes, craniotomy, needle aspiration) and administration of 100% oxygen

What a joy ride. I started working at the @theneuro_montreal 7 years ago- still a teenager and struggling with my mental health. I remember sitting in the Grandpre auditorium shortly after, attending my first formal #neuroimaging lecture, and being completely in awe of what we could achieve with this amazing non-invasive technology called #MRI. Then I joined forces with a team at the #DouglasInstitute to merge my passion for finding better treatment avenues in psychiatry with advances in brain imaging. Yesterday, I defended my #PhD in the very room that first inspired me to pursue this field. Words cannot express how grateful I am to my supervisors, Martin and Alan, and their respective teams for quite literally raising me and shaping me into the #scientist I am today. 50-year-old man with a history of HTN and DM presenting after an episode of confusion and bowel incontinence. Exam was notable for expressive aphasia and decreased attention. CT head with and without contrast showed multiple ring-enhancing lesions; MRI with and without contrast confirmed the lesions and further characterized them as having diffusion restriction. Labs were remarkable for leukocytosis. The lesions were suggestive of brain abscesses - atypical due to minimal surrounding edema. Further investigation was consistent with tuberculosis.
