An overseas patient that came to see us for a difficult aesthetic rehabilitation. Orthodontics was recently completed to improve the overall position of teeth but still very little upper teeth showed in the smile. In addition the anterior teeth were in an edge to edge relationship. In order to increase the amount of upper teeth showing, we did 14 upper conservative minimal and non-prep veneers and veneer onlays to open the bite. By opening the bite, the lower anterior teeth rotated back and gave us space to jump the bite. In addition 6 lower non-prep veneers were done to improve colour and the bite. Traditional crowns would definitely look better in this case but the patient would pay a steep biological price. When restorations are too bulky, it may not fit the lips. Here, the previous restorations were flared out and too long so they impinged on the lower lip. The patient had lip incompetence which is when the lips have a hard time closing over the teeth. A gum graft was done first at @johnstone_periodontist followed by a set of new crowns and veneers. While lifting the gums up can help in enlarging teeth, gum grafting will help to make teeth look smaller. The restorations were Emax by @ivoclarvivadent_na @thedentalwhisperer and bonded with Multilink also by @ivoclarvivadent_int. Ceramics were by @yuzomatsumura_rdt. Note the difference in the smile afterwards once the patient regained confidence. The shape of the lips are totally different and make sure you plan for this! As dentists, we should not get tunnel vision and focus inside the mouth. I’m not criticizing the previous work nor should we ever. The existing crowns were clinically and aesthetically ok but we must take into account the lips and face. 12 years ago, I was asked why I wanted to get into @penndentalmed. Like many applicants, I said I wanted to change people’s lives. Truth is, I had no clue how I was going to do that. Do people even care about teeth? Don’t people hate dentists?! 8 years ago, I was asked why I wanted to be a prosthodontist. Again, like many applicants I said I wanted to change people’s lives. Truth is, I still didn’t know how I was going to do that. Isn’t prosthodontics “a dying speciality”? I, like many people, had no clue what dentistry or prosthodontics could do. It is really up to dentists to educate the public of the power of dentistry and up to prosthodontists to educate others about the speciality. To many, dentistry plays a minor part in their lives. Routine checkups and cleanings. But to some, it DOES change their lives. About this photo: An amazingly transformative perio-prosth case where we saved every tooth. The existing teeth were very small and large spaces were present. Structurally most of the teeth needed crowns due to caries, wear and large failing restorations. A diagnostic wax up showed that the spaces could be closed by making the teeth more normal in size. The patient accepted the proposed plan. Hygiene had to be improved. The gums were treated and lifted up by periodontist @yeung7187. Afterwards, we restored everything with emax crowns. Ceramics by @yuzomatsumura_rdt. Note the supernumerary tooth on the lower! .... but most people would not notice it from a social distance. A nightguard and frequent recalls are required. A very pleasant and amazing patient to help out.
