CPR being one of the most important. Make a list of 3 skills you want to learn over the next 3 months, now make a plan and master a new skill each month. She has a rare heart condition called Bland White Garland Syndrome where the left coronary artery originates from the pulmonary trunk. Some other causes of sudden cardiac arrest in the young: Commotio Cordis, Brugada Syndrome, Hypertrophic Cardiomyopathy, Long QT Syndrome, WPW. Remember CPR basics for Adults: - 2 hands center of the chest, lower half of breast bone - At least 2 inches compression depth - 30:2 compression to breath radio - Stop each breath when you see chest rise - 100 to 120 compression rate per minute - No delay in compressions over 10 secs. Better outcomes if no delay. Black Widow vs. Brown Recluse Brown Recluse (Genus: Loxosceles) - Most active at night and not aggressive. Bites usually happen only when crushed or smashed. - Bite can sometimes be painful (sharp) however victims can be unaware of the bite until local tissue destruction/pain hours after initial bite. -Venom acts as a hemolysin and cytotoxin. This leads to tissue destruction and delayed wound heading. Rare and more so seen in children is severe systemic reactions leading to hemolysis, coagulopathy, renal failure, and even death. - Treatment is supportive with local wound care (cold compresses, elevation of the affected extremity, pain medication). Tetanus prophylaxis. Depending on the severity of the tissue destruction; wound debridement might be required. Monitor for cellulitis throughout treatment. No antivenom available in USA. Mexico has a recombinant sphingomyelinase D antivenom which does show to decrease local tissue destruction if given within 4 hours after the bite. Black Widow Spider (L. mactans mactans) - Bites are mostly defensive after being crushed. Some times are quite painful but at times the patient is unaware of the bite. Clinical suspicion should be utilized in patients with unexplained systemic muscle cramping. Peak of severity of symptoms usually around 8-12 hours with gradual improving symptoms  afterwards. - Venom is a neurotoxin which induces neurotransmitter release from nerve terminals. This leads to muscle spasms/cramping through out the body. Limited local tissue reaction. This can lead to abdominal pain which at times can mimic appendicitis or acute abdomen. In addition the neurotoxin can cause autonomic disturbances affecting blood pressure as well as a central nervous system. Case reports of premature delivery have been reported due to severe uterine contraction. - Treatment is local wound management in mild cases with tetanus prophylaxis. Severe cases require hospital admission (Children/Pregnancy most often) with IV pain medication and benzodiazepines. Antivenom reserved for respiratory arrest, seizures, uncontrolled hypertension, pregnancy, priapism.
