Today was day 3 of clinical! I followed a wound nurse and saw the most crazy, out of this world wounds. I had the opportunity to dress them and change them myself! It was quite the experience. Tomen nota amigos: -Intervenciones NO negociables e indispensables:
1 -Vendaje multicapa en esquema 24/7.
2 -Ejercicio descongestivo y funcional que estimulen la marcha de calidad.
3 -Higiene de la piel.
4 -Drenaje manual (y enseñarle autodrenaje al paciente)
5 -Higiene vascular (favoreces posturas que la gravedad favorezca)
6 -Apositos absorbentes de hidrogel y cambios frecuentes de los mismos, tanto como sean necesarios.
La mayoría de estos pacientes requieren control médico y estar en tratamiento y estables de otras posibles co-morbilidades existentes.
Recomiendo basarnos en la estrategia DOMINATE para el cuidado de la ulcera.
Debridación: retirar cuidadosamente el tejido necrótico, muerto, inviable o esfacelo residual para un lecho de herida sano.
Offloading: Manejo de la presión de la ulcera acorde a su fisiopatología de tal modo que se ejerzan presiones que favorezcan la sana fisiologia vascular del tejido y no al contrario.
Moisture: control de la humedad y el exudado.
Infección-inflamación: el manejo de procesos inflamatorios asociados tanto al exudado como a la colonizacion bacteriana de la piel.
Nutrición: juega un papel fundamental en la velocidad de curación de las heridas; habría que tener una labor interdisciplinaria con su especialista en nutrición clínica.
Arteriopatías: la valoración y tratamiento por parte del especialista en angiología; tanto a nivel venoso como arterial da un parámetro claro para definir qué tipo de ulcera enfrentamos, qué tipo de tratamiento es necesario y si en realidad hay algo que nosotros pudiésemos ofrecer (o no). Technics-tissue: Podrían elegirse otras técnicas de intervención avanzadas. (Uso de larvas para control de tejido necrótico, terapia de presión negativa, presión hiperbarica, etc)
Edema: el indispensable control del edema/linfedema por medio de las diversas técnicas de terapia compresiva.
El algorítmo DOMINATE en contraste con el clásico TIME (les toca investigarlo) me parece un poco mas amplio al considerar el abordaje de ulceras. -Lft. Today Pieter was allowed to sit in his big wheelchair for a moment and we helped him with his dinner because he is still to tired to eat by himself! The nurses are fallen in Love with his charming smile ( after a few days they had to learn how to deal with a complex patient like Pieter) and they use all the Monkeys to cheer him up and to distract him when they need to use a needle or something Pieter doesn't like. Dry wound dressing change.. why you have so many steps?! ✅ I sort of love opening a sterile field and using the forceps and gauze to clean wounds on mannequins not sure how I’ll feel once those are real live wounds and humans who can feel pain though . Anyone else enjoy performing dry wound dressing?
What a roller coaster ride I am on.....in this thing called life! A lead revision on my pacemaker turned into an entire pacemaker pocket infection, which then resulted in pacemaker removal, a huge infected wound that will require many weeks on a wound vac, and IV antibiotics.
As if that wasn't enough the right arm/shoulder now has an upper extremity DVT that has me on anticoagulation therapy. Further delaying pacemaker reinsertion surgery and not controlling my heart rhythm which is scary!
Although I try to show strength through everything, I've learned it's okay to feel scared, anxious, and sad. It doesn't mean I'm not strong, but just shows I'm human. Although my illness had taken so much, it will never take my hope for a better tomorrow.
It's taught me just how valuable our health is and how precious time is.
Life isn't beautiful because it lasts forever, it's beautiful because it's finite, precious, and fragile. Just because something is here one day that does not guarantee it will be here the next. It is ther greatest gift we've ever been given. Never squander it, never waste it, and don't ever take that for granted!
