Today, I had a non-English-speaking patient, seen with interrupter services, who was very upset with me for not being able to cure her chronic dermatitis. As much as I love how quickly I'm able to fix certain skin complaints, there are just as many problems we are not able to cure. It's hard for patients to grasp why we have fancy names for conditions, but can't explain how to best treat or why they happen. Believe it or not, this happens a lot in dermatology, and in medicine in general. Patients often think we have super powers and are able to treat and cure everything that walks through the door. Quite the contrary. I've found that setting expectations and educating patients are key to treatment compliance. However, when there is a lanuage and cultural barrier, its add another layer of complexity . I try to use analogies or simpify explanations. I write out my instructions, and translate to the appropriate language when necessary. Writing at a 5th grade or lower reading level, and including pictures are additional ways to increase patient comprehesion. But sometimes despite all of this, information still gets lost in translation . Have you ever had a challenging encounter with a patient? As you enjoy your time with family and friends this holiday, remember that some of us are caring for your loved ones in the hospital. This is my first time working through the holidays. It’s been especially tough because as an only child in a family of three, and with my birthday and my Dad’s birthday (which is today) being so close together, I’ve always spent Christmas with my parents. I miss them, and I wish I had more time to be with them, but such is the life of a physician. It’s a sacrifice, but a small price to pay to help those in need. Merry Christmas, everyone
