ɪɴᴀʀɪ . . . Cool case today using the Inari ClotTriever. This patient had severe ilofemoral DVT with an occluded IVC from a previously placed IVC filter. She presented with acute DVT and severe LE edema and venous insufficiency. We decided to thrombectomize her clot and restore flow to her LE venous system! . . . Here you can see all of the clot we removed (both chronic and acute) and restoration of flow from her IVC to her lower extremity. What a huge difference this device can make. . . . This device is a type of mechanical thrombectomy that uses a nitinol cage to macerate the clot and trap it in the nitinol filter that trails behind it. It requires a 14F sheath and good experience with IVUS and ultrasound access for good results! . . . ‍⚕️Have you ever seen any venous work before? It’s an emerging field and interventional cardiologists are starting to make strides in joining forces with IR to help patients feel better. Certainly a “wow-moment” in the echo-lab. This person is on the verge of getting a stroke! What do we see here? We see a TEE image. The left atrium is in the top, the left ventricle beneath it, in-between the mitral valve is to be seen. The dancing structure in the left atrium is a thrombus knocking against the mitral valve, not getting through, possibly due to a mitral stenosis. Size and shape of the clot insinuate that it has been formed in the left atrial appendage, possibly due to atrial fibrillation. Should the clot pass the mitral valve, entering the left ventricle, it might continue downstream through the aortic valve and lead to a cerebral stroke or a peripheral embolic event. ɪɴ ᴛʀᴀɴsɪᴛ ! . . . Here is a great example of a clot in transit! This is the term we use when we catch a mobile thrombus passing through the heart on echo. Here you can see the echodense and worm like features of a clot originating in the lower extremity veins and passing through the right side of the heart and likely into the lungs causing a pulmonary embolism. . . . There are many options to treat a clot like this. Mainly interventions are too late and the patient will develop a pulmonary embolism, which can be potentially fatal. Things like suction thrombectomy can be attempted however due to the location of the clot to the tricuspid valve this makes aspiration difficult and dangerous. One therapy that is showing promise is the use of a 135 cm EKOS (endowave infusion Catheter system) to infuse sound waves and TPA to decrease the clot burden while avoiding damaging the tricuspid valve. In this case embolism is almost guaranteed however if recognized early intervention can be potentially life saving. . . . Regardless of intervention the mainstay of treatment remains anticoagulation with IV heparin or subq lovenox followed by DOACs (direct oral anticoagulation) or warfarin. Also remember to preform an extensive history on patients presenting with DVT/PE- ask travel history, potential for cancer, recent surgery or trauma, family history, meds (i.e OCPs), miscarriages, etc. . . . ʜᴏᴡ ᴛᴏ ʟᴇᴀʀɴ . . . As July 1st approaches new fellows are eager to start their training. As a new fellow, you are going to be bombarded with new information from every angle. You will go from the top of the totem pole as a medicine attending to the bottom as a first year cardiology fellow. The first few months are going to be tough but you’ll make it if you stick to knowing the basics and don’t get bogged down with being advanced, just yet. . . . One of the toughest topics when you start cardiology fellowship is Echocardiography. It is often self taught and fellow driven and you pick up little things a long the way from senior fellows and attendings. The best way to approach echo in the beginning is taking the following steps: . . Learn the views and how to obtain them! . . Learn Doppler and the differences between them (Color, Pulse, Continuous) . . Learn the NORMALS- chamber quantification in particular (i.e what is the normal LA volume index) . . . ⭐️If you learn these three basics you’ll be ready to roll for the first year of fellowship. Some good resources to use are echocardiographer.org, university of Toronto TEE simulator, Kline Echocardiography review, Mayo and ASE board review videos, and the ASE guidelines on Doppler and Chamber Quantification. . . . I seriously used to hate echo...now I seriously love it. It’s such an amazing tool that gives you so much information and can be acquired in a pinch! . . . Good luck to all the new fellows out there! T a m p o n a d e! . . . So this video has been circulating any social media platforms and I thought I would share it! This is a video of a post mortem examination of a patient that died from tamponade from ventricular free wall rupture. As you can see the surgeon opens the pericardial sack to find a large amount of clot within the pericardial sack and is able to visualize the ventricular rupture. . . . Tamponade is a life threatening condition that can occur and escalate very quickly. The heart is contained within a sack called the pericardial sack which has elastic properties and allows the heart to exert pressure outward to fill. Just like rubber, if you stretch the pericardium to it’s elastic limit it becomes tense and unable to accommodate any further outward pressure so all pressure is exerted inwards. The chambers of the heart with the least amount of pressure (RA and RV) are at increased risk for diastolic collapse and impaired filling leading to hemodynamic collapse. . . . ‍⚕️Tamponade is a clinical diagnosis. Often on tests Becks Triad is what is seen (Muffled Heart sounds, JVD, and hypotension). It is important to remember that tamponade occurs because of the RATE of accumulation of fluid within the pericardial sack rather than the AMOUNT of fluid. You can have a large pericardial effusion without tamponade. . . . Key echocardiographic parameters we look for in tamponade is respiratory variation of the tricuspid and mitral inflow Doppler signals, distended IVC without collapse, ventricular interdependence (shifting of septum with inspiration), and early systolic collapse of RA and early diastolic collapse of the RV, inspiratory diastolic flow reversal in hepatic veins, among other things. . . . Recognition is key and trust in your clinical abilities to diagnose and treat tamponade as soon as possible! . . . Happy Wednesday! a L t E r N a N s! . . . To stick with the theme of tamponade (my last post) here is an example of electrical alternans! Electrical alternans is an electrocardiographic phenomenon defined as an alternating amplitude or axis of the QRS complexes in any or all leads. It is most commonly associated with a large pericardial effusion and impending threat of cardiac tamponade . . . To understand this finding you have to understand what the heart is doing when a large pericardial effusion is present. Normally the heart is surrounded by a small (trivial) amount of pericardial fluid within the pericardial sack. In conditions with large effusions the hearts does a swinging-like motion within that sack, swinging back and forth between EKG leads, leading to an alternation of QRS (or p or t wave) voltage as the heart swings back and forth within the sack, changing its distance from the EKG leads with every beat. . . . ‍♀️This finding is non specific and seen in only 5-10% of tamponade cases. However if you have a patient that you presume to be in tamponade with other clinical findings, an ekg can help point you in the right direction with this finding as well as low voltage. Use every tool you have to come to the right diagnosis. . . _____ Have you see electrical alternans in the setting of a pericardial effusion? ᗩTᖇIᗩᒪ ᔕEᑭTᗩᒪ ᗩᑎEᑌᖇᔕYᗰ . . . An atrial septal aneurysm (ASA) is a redundancy or saccular deformity of the atrial septum and is associated with increased mobility of the atrial septal tissue. ASA is defined as excursion of the septal tissue (typically the fossa ovalis) of greater than 10 mm from the plane of the atrial septum into the RA or LA or a combined total excursion right and left of 15 mm. . . . The prevalence of ASA is 2%–3%. ASA has been associated with the presence of a PFO, as well as an increased size of a PFO, and an increased prevalence of cryptogenic stroke and other embolic events. ASA has also been associated with multiple septal fenestrations, and this should be evaluated for carefully using color Doppler imaging on echo. . . . Many times this is an incidental finding and nothing is needed in terms of treatment. However if a patient presents with multiple cryptogenic strokes this aneurysm can be considered to be closed with the use of a ASD/PFO occlusion device. . . .
