"Time of death: 1:28 a.m." . . Early this morning, I pronounced a patient dead. It was my first time. Instagram & entertainment often portray a life in medicine to be glamorous, with perfectly curated scrub sets & pristine white coats. Often, the tough moments, especially those in residency training, aren't shown or shared. They're real, raw, & emotional. They don't fit into perfect little boxes. They have a way of grounding a person. . . The phone rang at 1:15 a.m. "I don't believe my patient is breathing anymore," the nurse said. "Can you please come check on her?" We exchanged looks & walked down to the room. I recognized the name - I had taken care of this patient for 2 weeks on my last medicine block & had gotten to know them pretty well. Young, jovial, but terminally ill. They were recently placed on in-patient hospice care. I felt gutted. . . We reached the room. Their blue lips were noticeable from the doorway. I took a deep breath. "Go ahead & do the physical exam," my senior resident said. I nodded. Placing the stethoscope on the patient's chest, I listened for heart sounds. There was no "lub dup" of the pumping organ. Next, lung sounds. No chest rise, but staticky breath sounds jarred me. I noticed the patient was still hooked up to a breathing machine. I unplugged it & removed the cannula from their nostrils. No breath sounds this time. I felt for pulses on their cool skin, appreciating the lack of bounding blood flow beneath my fingers. Pupils were fixed, dilated. I tapped on the eyes - no corneal reflex. . . We repositioned the patient into a more comfortable position. "Now call the family," said my resident. My heart sank. "You have to say the word "die" or "death". You cannot tell the family that their loved one "passed away."" I picked up the phone & dialed. The ringing lasted what felt like an eternity. Finally, an answer. . "Hello?" "Hello, this is Dr. Sind-" "Did they pass?" ""Yes. I'm sorry to inform you that they..... have.... died. I am so sorry for your loss." We spoke for a few moments in hushed voices, aware of the sacredness of the loss of life. The call ended & I walked back to the call room, feeling nothing & everything all at once.

Grateful- for the TRUST my patients, family members and friends put in me every single day. Grateful for the journey and everything it has taught me and continues to teach me. Grateful for every single inspiring physician and student I’ve met. And most of all I am grateful for the relationships I’ve made over the past few years.
