Tear Troughs! Placing filler under the eyes can help take away that tired look we all hate. Especially when we’re not even tired! This is my preferred method of filling: At the lateral lid/cheek junction where we can often see the boney prominence of the orbital rim, I like to place tiny alloquots down onto periosteum. Medially, in the true trough, I use a 25g cannula and place tiny little retrograde threads. I use VERY LITTLE product here; and I don’t extend past the mid pupillary line when depositing the product. I’ve definitely been burned in the past by delayed swelling here from too much product or product placed in the wrong plane. Less is always more for this procedure. And please make sure to stay aware of the important surrounding anatomy. There is always a big X over the infraorbital foramen when I am doing medial cheek and under eye treatments. . Questions? The malarpalpebral groove is the continuation of the tear trough in the outer half of the eyelid. Not everyone needs this area injected when using fillers for eyelid rejuvenation, but it can really improve results when it does become an issue.
