Pterygium, from the Greek pterygos meaning “wing”, is a common ocular surface lesion originating in the limbal conjunctiva within the palpebral fissure with progressive involvement of the cornea. The lesion occurs more frequently at the nasal limbus than the temporal with a characteristic wing-like appearance. The etiology is unknown. An increased incidence is noted in latitudes nearer the equator and in individuals with a history of increased UV exposure. Some studies have shown a slightly higher incidence in males than females, which may only reflect a higher rate of UV radiation. UV radiation, proximity to the equator, dry climates, outdoor lifestyle. Histologically, pterygia are an accumulation of degenerated subepithelial tissue which is basophilic with a characteristic slate gray appearance on H&E staining. Vermiform or elastotic degeneration refers to the wavy worm-like appearance of the degenerate fibers. Destruction of Bowman’s layer by fibrovascular ingrowth is typical. Hyphema is blood in the anterior chamber of the eye. It may appear as a reddish tinge, or it may appear as a small pool of blood at the bottom of the iris or in the cornea. While the vast majority of hyphemas resolve on their own without issue, sometimes complications occur. Traumatic hyphema may lead to increased intraocular pressure, peripheral anterior synechiae, atrophy of the optic nerve, staining of the cornea with blood, re-bleeding, and impaired accommodation. Secondary hemorrhage, or rebleeding of the hyphema, is thought to worsen outcomes in terms of visual function. The over-convergence associated with the accommodation to overcome a hyperopic refractive error can cause a loss of binocular control and lead to the development of esotropia. Accommodative esotropia is caused by accommodative convergence associated with hyperopia. As infants, the eyes are straight, but as they learn to accommodate to see clearly, the fusional divergence is not adequate and the child develops esotropia. Iridodialysis and traumatic cataract after firecracker injury. // Iridodialysis, sometimes known as a coredialysis, is a localized separation or tearing away of the iris from its attachment to the ciliary body. Iridodialyses are usually caused by blunt trauma to the eye, but may also be caused by penetrating eye injuries. Iridodialyses have been reported to have occurred from boxing, airbag deployments, high-pressure water jets, elastic bungee cords, bottle caps opened under pressure, water balloons, fireworks and various types of balls. Asteroid hyalosis with an intact posterior capsule. Asteroid hyalosis is a degenerative condition of the eye involving small white opacities in the vitreous humor. It is known to occur in humans, dogs, cats, horses, and chinchillas. Clinically, these opacities are quite refractile, giving the appearance of stars (or asteroids) shining in the night sky—except that ocular asteroids are often quite mobile. Ocular asteroids must be distinguished from the more common typical vitreous floaters, which are usually fibrillar or cellular condensates. The cause of asteroid hyalosis is unknown, but it has been associated with diabetes mellitus, hypertension, hypercholesterolemia, and, in certain animals, tumors of the ciliary body. In dogs, asteroid hyalosis is considered to be an age related change. The asteroid bodies are made up of hydroxylapatite, which in turn consists of calcium and phosphates or phospholipids. While asteroid hyalosis does not usually severely affect vision, the floating opacities can be quite annoying, and may interfere significantly with visualization and testing of the retina. While treatment of asteroid hyalosis is usually unnecessary, vitrectomy may occasionally be indicated, for both diagnostic and therapeutic purposes.
