On the other end of the spectrum, we (clinicians) and our words also have the ability to elicit nocebo effects leading to increased pain symptoms coupled with negative behaviors (fear-avoidance). The question becomes if we are placebos, then ethically how much will we maximize the effect without unnecessarily conditioning patients to us as clinicians or unnecessary interventions. Any other practitioners in the Bay Area interested in attending? - Top Picture: Low end of the curve is the threshold stimulus required to cause a desired adaptive effect. Upper end is a point of diminishing returns - beyond this point, further application of stimulus yields no increased benefits or perhaps could have detrimental effects. (Adapted from Plisk, 2003). - Bottom Picture: The second slide expands on this thought. We need to be eliciting a sufficient stressor in rehab to evoke adaptation. If rehab remains low-level then we can almost guarantee loss of conditioning and baseline fitness without sufficiently developing the athlete back to pre-season status. Low-level exercise is dependent on the athlete's individual characters and injury type.
