The other day I was strapping into my snowboard when my friend asked, “Hey Taylor, do you believe in stretching before or after exercise?” “Well, the research doesn’t show any long term benefit to it, so unless that changes, no.” My other friend chimed in, “but isn’t half of your job telling people to stretch?” I replied, “Not if I’m an evidence based clinician.” . The public perception of PT is a discussion for another time, but I think the topic of stretching raises an important distinction that I make in my clinical reasoning: is my goal to drive tissue adaptation or manipulate sensation? . ⛷ We know that stretching for 3x30” might have a short term effect on ROM or symptoms, but likely doesn’t have any long term effect on tissue structure. (Consider the literature in pediatrics and neuro that suggests splints and orthoses to prevent/mitigate joint contractures be worn for a minimum of 6 hrs a day.) . ❄️ Even so, there can be benefit to providing the patient with a movement that temporarily alleviates their symptoms. I have nothing against prescribing a patient with stretches to perform PRN as long as they understand it’s a temporary, “feel good” option that’s addressing their symptoms, not driving tissue adaptation. From the way I understand the literature, it’s the icing on the cake, not the meat and potatoes of therex. . ☃️ How do you explain stretching to your clients/patients?
