Vasectomy, a widespread form of birth control, is associated with a small but significantly increased risk of prostate cancer over the long run, according to a comprehensive analysis of Danish nationwide registries.⠀⠀

That said, the absolute increased risk of prostate cancer following vasectomy is...small and similar to the increased breast cancer risk in women following oral contraceptive use.⠀⠀

The study was published online May 23 in the Journal of the National Cancer Institute (JNCI).⠀⠀

In an accompanying editorial, lead editorialist Lorelei Mucci, MPH, Harvard T.H. Chan School of Public Health in Boston, and colleagues remain unconvinced and conclude that "it remains unclear whether vasectomy is or is not a cause of prostate cancer."⠀⠀

Have you ever pooed yourself, and been sick at the same time, while having a shower ? I have. Who says men can’t multitask?

Well last night wasn’t fun, neither was this morning, or most of today. Putting my positive head on though, i did at least avoid pooing myself during the marathon. By today’s standards this is kind of winning i suppose. Just the 4 episodes of being sick was enough. Sorry fellow runners.

I must thank everyone that ran with me today. The fire fitness crew and the running club here in Bangladesh welcomed me with open arms. Sadly i wasn’t my usual enthusiast self this morning. Instead i came with the fun package of food poisoning (again), and a rather grumpy overtired appearance and attitude.

At midnight last night - after trying to eat dinner at 21:00, and then squirming around in my bed uncomfortable for a couple of hours, the horribly familiar feeling that food poising was looming began to set i. I spent most of the night in hot sweats, or shiveringly cold reaching for the aircon button again.

This was not the best way to start a marathon. With 20 or so enthusiastic runners waiting at the lobby at 05:00 this morning i slowly counted down the hours before it was time to either cancel and let many people down, or man up, and take another marathon on with diarrhoea, vomiting and the overall feeling of being incredibly faint, fragile and weak. Without the food poising, the fact my last meal was 31 hours earlier was not a good start.

I feel very sorry for all the lovely people that support today. I was virtually mute, and desperately concentrating on getting the job done, plus not pooing myself. Thank you patient and lovely people.

I am now going to sleep in the hope my body now knows what to do. Come on probiotics kick in. I have a marathon to run in 48 hours time.

Enzalutamide Joins Initial Tx List for Metastatic Prostate Cancer⠀

The oral androgen receptor inhibitor enzalutamide is an effective first-line treatment option for men with metastatic prostate cancer, conclude researchers reporting the phase 3 ENZAMET trial.⠀
⠀
Enzalutamide significantly improved overall survival (OS) in comparison with a conventional nonsteroidal antiandrogen (NSAA) when both were added to standard of care (SOC) in this setting.⠀
⠀
SOC was either a luteinizing hormone-releasing hormone analogue or surgical castration. ⠀
⠀
Notably, the SOC also included early use of docetaxel in about 45% of patients, which allowed investigators to also report on this "subgroup of interest." ⠀
⠀
In total, the investigators randomly assigned 1125 men with metastatic prostate cancer to receive SOC plus either enzalutamide or a conventional NSAA (bicalutamide, nilutamide, or flutamide).⠀
⠀
The overall survival rate at 3 years, which was the primary outcome, was 80% in the enzalutamide arm and 72% in the NSAA arm (hazard ratio [HR], 0.67; P = .002).⠀
⠀
At 33 months, 64% of the enzalutamide arm were still receiving their assigned study treatment, in comparison with only 36% of the NSAA arm.⠀
⠀
Enzalutamide also substantially improved time to progression compared to NSAA (P < .001).⠀
