I’ve been a little MIA on here. With nationals last week, midterms, papers and everything, I was trying to stay focused and not get too behind! ‍♀️ . I hope everyone is having a good week so far! I think the hardest part of the semester is over and I can start thinking about exams and getting ready to study. It’s coming up faster than I’m ready for lol. ‍♀️ . In other news, clinicals are going super well and I’m really starting to develop a solid routine for the day in regards to time management and practical skills. It’s a good feeling to put everything I’m learning together into practice as the end of this semester is getting closer! Happy #MedicalMonday! ‍⚕️ This weeks topic is acute kidney injury (AKI) . AKI occurs when there is a rapid decrease in renal function, which can happen in just days or weeks. This causes a build up of toxic products in the blood (azotemia), and often a reduction in urine output. ⬇️ . Symptoms can include a reduced urine output, swelling in legs, ankles or feet, fatigue, shortness of breath, confusion or chest pressure/pain. . Diagnosis is based on, but not limited to, lab tests of renal function, which include serum creatinine and blood urea nitrogen (BUN). Elevated levels mean that the kidney is damaged, and not working as well to excrete these substances. ❌ . People who usually have AKI are people with chronic kidney problems, hypertension or peripheral artery disease. ☝ . Fluid, electrolyte and acid-base disorders develop very quickly. The most serious is hyperkalemia (high levels of potassium) and fluid overload, which causes edema. . Patients with AKI will often have peripheral edema, meaning a build up of fluid in their legs and feet, and sometimes up to the hip if they are in bed most of the day. . Pulmonary edema is a serious condition where fluid builds up around the lungs, making air entry into the lungs and breathing very difficult for the patient. . Treatment of AKI usually involved restriction of fluids, sodium, phosphate and potassium intake , administration of duretics (to get rid of the excess fluid causing edema) and any other treatments for the causes of AKI, such as an antihypertensive. ⬇️ . IV Insertion . Swipe for part 2, and yes the videos are obviously sped up ‍⚕️ . FINALLY learned this skill. The veins were massive so we couldn’t miss and it was on a mannequin but still ‍♀️ . Not bad for my first time! I needed to advance the catheter a bit more before releasing but I feel like it will slide a little easier on a real human lol . A few quick tips: Make sure you choose the right catheter size for your patient. Choose the right vein by feeling for it on the patient. It should feel bouncy, and it’s easier to feel with clean, bare hands opposed to gloves! But always put gloves on before actually inserting the needle. Ideally, start the IV in the most distal veins and work proximally. Apply the tourniquet 4-6 inches from your chosen insertion site. Stabilize the vein with your non dominant hand and insert the needle until you see flashback (blood), release the tourniquet, advance the catheter, then release the needle.
