As July 1st approaches new fellows are eager to start their training. As a new fellow, you are going to be bombarded with new information from every angle. You will go from the top of the totem pole as a medicine attending to the bottom as a first year cardiology fellow. The first few months are going to be tough but you’ll make it if you stick to knowing the basics and don’t get bogged down with being advanced, just yet. .

One of the toughest topics when you start cardiology fellowship is Echocardiography. It is often self taught and fellow driven and you pick up little things a long the way from senior fellows and attendings. The best way to approach echo in the beginning is taking the following steps:

Learn the views and how to obtain them! .
.
Learn Doppler and the differences between them (Color, Pulse, Continuous) .
.
Learn the NORMALS- chamber quantification in particular (i.e what is the normal LA volume index) .
.
.
⭐️If you learn these three basics you’ll be ready to roll for the first year of fellowship. Some good resources to use are echocardiographer.org, university of Toronto TEE simulator, Kline Echocardiography review, Mayo and ASE board review videos, and the ASE guidelines on Doppler and Chamber Quantification. .

I seriously used to hate echo...now I seriously love it. It’s such an amazing tool that gives you so much information and can be acquired in a pinch! .

I love being an Emergency Medicine doctor and I cannot imagine doing anything else!

However no job or speciality is perfect. There are always going to be things we do not like or things we wish could be different.
.
As medical students are choosing their specialties, it is important to consider all aspects of each speciality!
.
➡️ No continuity of care- I will probably never see most of my patients again. This makes it very difficult to build a long term relationship with patients. Many days I am left wondering what happened to that patient, did they follow up with their doctor, what was the outcome after discharge or transfer.
.
➡️ Multitasking - At any given time, we could be managing multiple critically ill patients.
.
➡️Erratic schedules- No real schedule. Emergency Departments are open 24 hours, 7 days a week. Which means a doctor always has to be working. It is difficult to have a set schedule with emergency medicine. You can be working nights one day and then jump to a string of days then back to a midday shift. Most of us do not have a set schedule, any hour or day is fair game.

➡️ Working holidays, weekends, and nights. You may end up missing events, birthdays, and much more m. Most of us have to work some weekends, holidays, and nights.
.
➡️Unpredictable. You do not get to choose who you work with, what patient you will see, or what type of case you will get! It is important to be able to get along with the entire team and be ready for anything that walks through the door.
.
➡️ Hospital employees. Most EM doctors work for hospitals or groups. I love this about EM but this may be difficult for people who want to have more autonomy. Most EM doctors do not really have their own private practice or their own clinic.
.
➡️ High stress, especially at a trauma center.
.
.
No doubt this is a tough job but I love it! There is nothing else I would rather spend the majority of the rest of my life doing!

Man! It’s been a quick three years since starting general fellowship and all of a sudden it’s over. I remember my first day as a fellow, scared out of my mind. I was so worried about trying to learn this crazy field of cardiology. There were times in fellowship where I didn’t think I could make it, other times where I didn’t think I was strong or good enough to be a cardiologist. However, with the encouragement of my family and colleagues I found my grove and passion for the field. It’s crazy to me watching the first year fellows go through it now and seeing the same feelings of hardship I went though. All I say to them is keep working hard and it will all come together. I’m so blessed to have made it this far and I definitely couldn’t have done it without the help of my co fellows, attendings, and most importantly my family (especially my wife). Cardiology can be a demanding field. This graduation is for them as much as it is for me. .

Can’t wait to see what the next chapter brings, but either way, I’m so glad I picked this path and Ives truly enjoyed the ride. Interventional cardiology here I come! .

☠️Ventricular fibrillation (VF) is the most lethal and worrisome heart rhythm. It cannot support life and is associated with hemodynamic collapse. It is usually provoked by something (dugs, toxins, ischemia, congenital etc) and can often be associated with a substrate (scar- VT mediated VF). .

Cocaine is one of the common toxins that has been associated with VF arrest. Cocaine puts you at a 7x increased risk of having a life threatening arrhythmia. Cocaine it self causes vasospasm, increased catecholamines, tachycardia, and coronary thrombosis- a vicious circle of increased oxygen demand with a decrease in supply to the myocardium. Combine this with many cocaine users drink heavily while using putting them at risk of forming a highly toxic compound called Cocaethylene that actually increases your risk of death 25x fold!! .

The best way to get you patients to listen to you is to show them the hard facts. Let them know the risks of their behavior but also give them the warning signs to help them recognize when to present so appropriate medical care can be initiated in a timely manner! .

This was an incidental finding. The CXR shows a displaced right midclavicular fracture. There is also diffuse, extremely high density opacification of the lower and middle zones of both lungs, prominent linear and reticular opacities in the upper zones.

The CT images show diffuse interstitial and alveolar calcification, most severe in the basal lower lobes and right subpleural lung.
