Maci Surgery: Week One Is now in the books! We survived. Cleared to bear 20lbs of weight on the “bad “ knee, while on crutches and with brace locked. Still struggling to find the sweet spot between normal pain and “I’m dying slowly” pain. Quad muscle is just a limp noodle, and makes moving around lots of fun. I’m SO very impressed by the clean incision and low swelling. Considering they did work in four different areas, I’d say it looks amazing! Leg can bend to around 40 degrees, after a session of stretching, so we are right on target there. Infection was a concern, because of my suppressed immune system, but praise be! God is allowing it to heal nicely. You have to know what your looking at in order to fix the problem. Varus Thrust Gait. With stance phase, you can see BOTH knees abruptly shift laterally. Step, load, shift laterally. But why Patient had bilateral lateral patellar release surgeries. Basically trying to decrease pressure/pain at the knee. Makes sense , look at that gait ‍♀️ BUT did it help Our patient here has been in pain for YEARS but now with foot pain. Diagnosed with posterior tibial tendinitis, has had shockwave therapy, orthotics etc and she can’t go to a concert without being in pain for weeks. Makes me sad and determined. Here’s the deal: 1️⃣ The varus thrust gait is causing a hyper extension and lateral shift at the knee 2️⃣ Causing the pelvis to dump forward....anterior pelvis tilt 3️⃣ Causing overload of the quads 4️⃣ Resulting in poor hip extension, all hamstring activation 5️⃣ Wrapping things up with internal rotation of the lower extremities, and the poor little posterior tibialis getting the beat down Treatment: 1️⃣ I went DNS heavy here. Breathing, postural awareness of her knee extension, how to contract her adductors driving her out of her anterior tilt and engaging those glutes Lots to do here but I’m excited. There’s just too much here not to be .
