Scopolamine is an alkaloid derived from some species of nightshade (usually Scopolia) that acts as a muscarinic receptor antagonist. It is used primarily to treat motion sickness, but is also useful in reducing nausea and vomiting after surgery and as part of a pre-medication cocktail before surgery. It was first used in the early 20th century as an anaesthetic, but more effective options have since become available. Scopolamine targets the muscarinic acetylcholine (Ach) receptors of both the central and peripheral nervous system, where they act as an antagonist, blocking the activity of Ach in some areas of the nervous system. Compared to some anticholinergics, scopolamine is fairly non-selective, but there is some evidence that it will preferentially target the m1 muscarinic receptor. Blocking the muscarinic receptors can block the signals between the vagus nerve and vomiting centre that induce vomiting and motion sickness, making scopolamine a useful anti-emetic. It is often used as a transdermal patch, which can increase compliance in patients who feel too nauseous to swallow pills. Scopolamine can also be used to reverse Ach poisoning (by muscarine, nerve gases etc.) but atropine is the preferred treatment option for these. Due to its general non-selective antagonism of Ach, the most commonly used neurotransmitter of the parasympathetic nervous system, scopolamine can have a wide range of side effects. The most common of these include sedation, dry mouth (useful before surgery) and dilated pupils, though bradycardia (and at high doses, tachycardia), hallucinations, itching and urinary retention can also occur. Because of its hallucinogenic effects, scopolamine has the potential for recreational abuse, but is used less commonly than other hallucinogens due to its unpleasant effects. Scopolamine is contraindicated in those with glaucoma and intestinal obstruction due to its effects on smooth muscle, and should not be used by pregnant women. It should not be taken with other anticholinergics, such as atropine, or with muscarinic agonists (except as an antidote). .
