Can only fall asleep on your side? But have shoulder pain on that side? Here is a way that physiotherapist Clyde teaches his clients to sleep on your painful shoulder. In side-lying on your affected side, place a pillow under your side from just above the pelvis to below the shoulder, and the other(s) at the head. Ideally the spine should be straight from head to pelvis, to avoid the "swinging bridge position" where one side of the spine gets compressed and the other stretched. A pillow between the knees can also help stop the spine from rotating. Placing the pillows in this manner helps create a gap to relieve pressure off the painful shoulder. ⠀⠀

The shoulder joint has an impressive amount of mobility and allows us to move in many different ways. In sport, we often rely on the ability of the shoulder joint to rotate around the socket. As a physiotherapist, we are interested in the ability of the shoulder to rotate. ⠀⠀

Here, Heather is demonstrating both how she assesses internal rotation in the clinic and a home test which patients can use. Notice how the patient’s right forearm is not able to rotate as far down. ⠀⠀

To check your inward rotation at home, bring your elbows up to 90 degrees with your palms facing down. Keep your elbows at 90 degrees while you rotate your hands toward the floor. Notice if there are any differences between the two sides. ⠀⠀

Improve the hip mobility with Dan's exercise from Monday, and then get the control with this exercise. Be sure to keep the spine in neutral. In Heather’s blog post (link in bio!), we discuss the importance of keeping the elbows in line with each other in the tennis serve (Trophy Posture). This balancing concept is also important in many other sports, for example swimming and throwing. The Archer’s Pull exercise is a great way to practice this posture. It incorporates mobility work for the thoracic spine and strengthening for the shoulder. The focus of the exercise is keeping the elbow in line with the shoulder with no shoulder hike.⠀

Over the next week, Heather (physiotherapist at our Chan Gunn) will offer some simple tips on shoulder rehab. ⠀⠀

A dynamic warm up is key before engaging in activity. The band separation drill is a great addition to any upper body warm up. It helps activate muscles from the shoulder blade down to the hand.⠀⠀
These exercises can be done laying on the ground, or standing with the back against a wall. ⠀⠀
- Start with the arms shoulder width apart holding onto a light to medium resistance band. ⠀⠀
- Separate the band, emphasizing smooth, slow and controlled motion. ⠀⠀
- Move the band to start at different points of the body to bias different muscle groups. ⠀⠀
- Heather recommends using the forehead, shoulder and bottom of the ribs as three target areas.⠀⠀
- Incorporating transition motions between different zones is also helpful. Separate the band at the forehead, keep tension on the band as you slowly and smoothly move it down towards the bottom of the ribs. Release tension and repeat in the opposite direction. ⠀⠀
