☀️ Eh oh Po!
I found a Teletubby in a lymphangioma.
My work here is done.
Time for tubby toast

A lymphangioma is a malformation of the vessels that carry lymph around the body. These vessels can grow to become dilated cystic fluid filled structures within the skin and other regions of the body. This can present as a large soft tumor.
Po’s head is one of these dilated lymphatic vessel: lined by an endothelium and filled with lymphatic fluid. The eyes and mouth are artifacts of tissue preparation in this fluid.

1. Brenner tumor
The head of this giraffe is composed of urothelial-like cells (the cells that normally line the bladder & ureters. However, the strange thing about them is that they were found in the ovary. by @DrAldehyde via Twitter
2. Endocervix
The region of the neck of the uterus that is continous with the inner lining of the uterus. The transition between endocervix and ectocervix undergoes a lot of change during life and is the region most susceptible to cell changes that cause cancer. by @jestevamd
3. Smooth muscle
Smooth muscle is a muscle that contracts involunatrily and usually around tubes to move stuff through them or maintain pressure. They are ‘spindle’ shaped cells (fat in the middle and tapering at each end) with a centrally located, cigar-shaped nucleus. This means that the cells look different depending on if you slice them along their length (longitudinal section) or across their width (transverse section). You can see examples of both in this image. by @onlyfornursed via FB
4. Cervical vertebra
The pink stuff is the cancellous/spongy bone of the vertebra and the white space (that makes the shape of the giraffe) is the medullary cavity where the bone marrow lives. The irony that a cervical vertebra is a bone in the neck is not lost on me! by @ihearthisto
5. Mammary Gland
This image shows a biopsy obtained from a mammary gland during pregnancy. You can tell because the breast contains a high ratio of glandular tissue (the many purple circles) to connective tissue (the pale pink stuff surrounding the glands). In addition, the ducts of this breast are developed and contain some secretory product (milk). The entire giraffe is a formed from a lactiferous duct surrounded by connective tissue.

Lancisi Sign- Severe Tricuspid Regurgitation
.
.
.
The tricuspid valve is the three leaflet AV valve that separates the right atrium from the right ventricle. A common valvular abnormality from this valve is Tricuspid Regurgitation (TR). TR is a relatively common abnormality. Since this condition is frequently asymptomatic and may not be detected on routine physical examination, it is often diagnosed solely by echocardiography. Usually, there are no specific signs or symptom and often times related to pulmonary HTN. .
.
.
In case of severe TR, giant C-V waves or the Lancisi sign can be found on the jugular venous examination (seen in the video above). With increasing tricuspid regurgitation, there is an increased backflow of blood to the right atrium during systole. In patients with severe tricuspid regurgitation, the V wave of tricuspid regurgitation merges with the C wave forming a single prominent C-V wave that is often mistaken for the large carotid-pulse wave of severe aortic regurgitation. .
.
.
Other signs and symptoms include painful hepatosplenomegaly, ascites, and peripheral edema. Auscultation reveals a loud first heart sound with a loud pulmonary component of the second heart sound, an apical mid-diastolic rumble, and a holosystolic murmur at the left lower sternal border. Generally, tricuspid valve disease occurs secondary to left-sided heart valve disease, in particular mitral valve disease (i.e., functional TR). It is a marker of adverse outcome, and patients with moderate/severe TR have a worse prognosis. Primary TR is treated surgically if severe, and the patient is symptomatic. However, during concomitant left-sided heart valve surgery, a moderate/severe secondary TR with either raised pulmonary artery pressures or tricuspid annular dilatation should also be treated. .

A-Airway
Anesthesia is all about providing a patent, protected airway. Patients deemed to be at risk of aspiration require a cuffed endotracheal tube. If not at risk of aspiration and no requirement for muscle relaxation, we can use an LMA
Prior to induction, ensure you have the following equipment readily available.
Two different laryngoscope blades usually curved + straight
A selection of oral airways (see our previous video on sizing airways)
Two different endotracheal tube sizes - check patency of each cuff beforehand
Adjuncts to intubation eg stylet/ bougie
Consider backup LMA as per DAS guidelines https://das.uk.com/guidelines/das_intubation_guidelines

—The Future of Medicine—
.
.
.
Already so much has changed from when I went to medical school. Yes we had computers and google but never did I think most of the education would come ONLINE (man I feel
Old). Even as a fellow, just a few years removed from medical school, I now I use so many online resources like eBooks, medical video streaming, YouTube, simulators (from industry), apps, etc. .
.
.
The funny thing is...this is only the beginning. Already down the pipeline 3D and holographic learning tools are being developed (AND USED) like the ones you see above. This freaks some people out but as a Proceduralist this makes me so excited. We will have the opportunity to produce an artificial patient (modeled EXACTLY) after a real patient and test our planned strategy and devices PRIOR to the procedure. .
.
.
Already with the corindus robot allowing for robotic PCI we have entered the realm of robotic/AI treatment in cardiology. Does that freak me out? A little. But at the end of the day cardiology has always been a field that embraces new technology and thus often times is on the forefront of medical Breakthroughs

I used to have a reoccurring nightmare that I couldn’t get into medical school no matter what I did. I would gasp and wake up in a sweat. The stress of getting into medical school was too much. But one day, something really weird happened. I woke up gasping as usual. Then I started to slowly recognize my bedroom as the fog and panic faded away. I was in my tiny apartment in Benicia...
And I had anatomy class at my medical school in 30min.
I was a medical student already.
Wow.
I made it happen.
But my pre-Med anxiety was still so palpable. My gratitude was overflowing in my heart as I happily jumped out of bed to get ready.
Imagine how glorious that felt for me.
It can be you one day soon too.
Holy moly! Just imagine it.
Overcoming low scores is possible and it’s REAL.
