Twin-to-twin transfusion syndrome (TTTS) can affect identical twins who share a placenta (monochorionic twins). ~ TTTS happens when there is an imbalance in the placental blood vessels that connect both twins. If the blood doesn't flow evenly between the twins, one gets more blood, called the recipient twin, while the other gets less blood (the donor twin). ~ The uneven blood flow results in the recipient twin growing too big, thanks to the extra nutrients and fluid he's receiving, and the donor twin growing too slowly. ~ The extra fluid the recipient twin takes in can put a strain on his heart. His body will try to get rid of the fluid by producing more urine. As a result, he will have too much amniotic fluid around him, while his twin will have little or none. ~ The recipient twin and the extra fluid can squash the donor twin against the wall of the womb (uterus). ⚜️TRANSTHALAMIC PLANE AND THE BIPARIETAL DIAMETER⚜️ ‍The biparietal diameter (BPD) was one of the first sonographic parameters used to estimate fetal age. It has been shown to be a reliable predictor of gestational age in the first half of pregnancy, being the most accurate between 12 and 18 weeks (+-1.2 weeks)! ‍As pregnancy progresses, the BPD loses its power to correctly predict gestational age, with a margin of error of +- 3.2 weeks at 36 to 42 weeks. ‍It’s important to highlight that head measurements that take into account shape of the fetal head - namely, the correct BPD and HC - are more accurate than the BPD alone! ‍The Biparietal Diameter represents the widest transverse dimension of the fetal head!
