RETINAL DETACHMENT REPAIR with vitrectomy . Viewer discretion advised. . . #Vitrectomy is the surgical removal of the vitreous jelly and the internal approach to repairing a #detachedretina. Typically suitable for patients 50 or older with some exceptions. . . The key is thorough shave and removal of vitreous 360 but especially around the retinal breaks, which in this patient were inferior (superior in the view). Scleral depression (indentation) from the #scrubtech brings the far peripheral retina into view for close shave *anterior* to the tears which relieves the traction and is crucial for single surgery success. . . Once vitreous is out, perfluoron injection flattens the retina. Scleral depression with one hand again brings the far periphery into view while using an illuminated laser in the other hand to surround the retinal breaks with white rows of argon endolaser burns that evolve into a barricade/seal during the post-op period. . . Air-fluid exchange drains the perfluoron and fills the vitreous cavity with an air bubble. Alternate draining from the site of the retinal break and from above the optic nerve. . . The #sclerotomy sites are sutured and the air is exchanged for a gas bubble, which lasts 1-3 months depending on the type and concentration chosen. Air is vented out through a chimney or an empty needle. . . Actual length of surgery ~45min. . . Patient is alert and comfortable with local anesthesia and intravenous sedation. .

The anterior layers disintegrate into thin fibrils with free ends floating in the anterior chamber.
