Inhaled anesthetics are gases or volatile liquids given through a mask or a tracheal tube. The anesthetic goes from the lungs to the blood, and on to different parts of the body. Anesthetics with a high *lipid* solubility accumulate in the body fat, increasing potency. Anesthetics with high *blood* solubility bind to blood proteins and tend to have slower onset and recovery.⠀⠀⠀⠀⠀⠀⠀⠀⠀ .⠀⠀⠀⠀⠀⠀⠀⠀⠀ ️ Let’s start with nitrous oxide, which acts as an NMDA receptor antagonist. It has a very low solubility in blood and lipids, so induction and recovery from anesthesia is rapid, but its effects are weaker, so it can’t achieve anesthesia on its own. However, at lower concentrations it has an analgesic effect, so it’s often used in dentistry. Nitrous oxide also has a “second gas effect”: when combined with other anesthetics, it lowers the therapeutic dose required for those other agents. Nitrous oxide can cause expansion of trapped gases in closed body cavities, so it’s contraindicated for people with pneumothorax or bowel obstructions.⠀⠀⠀⠀⠀⠀⠀⠀⠀ .⠀⠀⠀⠀⠀⠀⠀⠀⠀ ️ Next, we have the halogenated inhalational anesthetics like halothane, enflurane, methoxyflurane, and desflurane. These act as NMDA receptor antagonists, but also as GABA receptor agonists. Induction and recovery from anesthesia can vary, since these medications have different blood solubility, but they are more lipid-soluble than nitrous oxide, so are more potent and can induce anesthesia. These medications all cause vasodilation, cardiovascular depression, and respiratory depression. They will increase the blood flow to the brain, which can lead to increased intracranial pressure.⠀⠀⠀⠀⠀⠀⠀⠀⠀ .⠀⠀⠀⠀⠀⠀⠀⠀⠀ ️ The last key concept is that the respiratory system normally responds to hypoxia by increasing the respiratory rate, but inhaled anesthetics diminish this response. As a result, it’s hard to notice hypoxia, because we don’t have signs like tachypnea. It’s time for another pharmacology post! Today, we’re going to be looking at laxatives—specifically, osmotic laxatives!⠀⠀⠀⠀⠀⠀⠀⠀⠀ .⠀⠀⠀⠀⠀⠀⠀⠀⠀ Laxatives are medications used to treat constipation by making it easier for stool to pass through the body. Constipation causes people to pass small, hard stools, or experience difficulty initiating bowel movements. It can occur due to a poor diet or a malfunction within the GI tract itself, although, up to 60% of chronic constipation does not have a clear cause. Normal stool frequency is usually *at least* 3 times per week for someone on a typical Western diet.⠀⠀⠀⠀⠀⠀⠀⠀⠀ .⠀⠀⠀⠀⠀⠀⠀⠀⠀ There are several different types of laxatives, all with slightly different mechanisms of action. Osmotic laxatives include ion-forming compounds like magnesium sulfate, magnesium hydroxide, sodium phosphate, and indigestible alcohols and sugars, like polyethylene glycol and lactulose.⠀⠀⠀⠀⠀⠀⠀⠀⠀ .⠀⠀⠀⠀⠀⠀⠀⠀⠀ Magnesium cations and phosphate anions are not well absorbed by the body, just like the indigestible alcohols and sugars. So as they move through the intestines, they draw more water out of the cells in the intestinal wall via osmosis and increase the amount of water in the lumen. This increases intestinal motility which pushes the stool through the GI tract and also helps mix the stool with water.⠀⠀⠀⠀⠀⠀⠀⠀⠀ .⠀⠀⠀⠀⠀⠀⠀⠀⠀ Besides treating constipation, these agents can be used to cleanse the bowels before procedures like colonoscopies or surgeries, kind of like using a drain cleaner to clean dirty pipes. The downside is this could cause a lot of fluid loss, leading to dehydration.
