This 47-year-old woman who works in a high powered job in Boston came to us with concerns about looking tired and sad when she was not. This was certainly not in her best interest professionally as the job required her to be in front of a lot of people making presentations. She was especially bothered by the appearance of her neck. I provided her with several surgical options and she decided to have her eyelids, mid-lower face, and neck all addressed at the same time. She saw the advantages of having everything match artistically. With my multilayer customized SMAS facelift, we lifted the area around her mouth to treat her sad frowning appearance in addition to contouring the cheek and jawline. We performed a neck rejuvenation surgery with subplatysmal fat removal (removing a pocket of fat that she had inherited from under the muscle). This is fat that would not have been possible to remove with liposuction. In fact, liposuction usually makes a problem like hers worse. We released the submental crease under her chin to further smooth out the chin and neck area. Liposculpture/fat injections were done to fill out deflated areas of the face including the area between the jowls and the chin. Fat injections were also done to augment the chin in an effort to create a better artistic natural facial balance. As you can see the end result is a natural rejuvenation. These photos show a woman in her early 60s before and six weeks after upper and lower blepharoplasty surgery. After surgery she looks more youthful and better rested without looking fake or overdone. She is only six weeks after surgery and still has some swelling, which will continue to improve over the next several months.⠀ ⠀ Her left eye is more prominent than the right due to differences in the bony framework around her eyes (orbital asymmetry causing relative exophthalmos). The difference in bone structure makes the left eye look “bigger” than the right. We decided not to address this issue, as it would have required more extensive surgery. This is the excess skin that gets removed as part of a blepharoplasty or upper eyelid lift. Traditionally, upper blepharoplasty used to involved aggressive removal of skin, some orbicularis muscle and fat, which ended up leaving patients looking too hollowed out and skeletal as they aged. Nowadays, surgeons trained in newer methods take precautions to avoid these issues. In most cases, only skin needs to be removed, but in select cases, upper eyelid fat may be repositioned.
