It’s no secret that the advent of dermal fillers has changed the entire field of aesthetics, from a surgery-based treatment to office-based treatment. But there seems to be much confusion about the amount of filler it takes to reverse aging changes. As @newbeauty demonstrated, if you take an off the shelf dermal filler like Versa or Voluma and inject it into a teaspoon, it would take five syringes to fill just one teaspoon! Most people are surprised by this mathematical fact. One syringe of filler is 1cc and one teaspoon is 5cc’c. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ The amount of filler needed to reverse aging and enhance beauty really depends on the age of the person, how well they have aged, and how well they have maintained their skin and face with aesthetic treatments. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Here are my starter numbers for those who are under 40 years old: Forehead 1cc. Temples 1cc. Brow hollowing 1cc. Lower lid hollowing 1-2 cc. Cheeks 2cc. Jaw contour 2cc. Smile lines 2cc. Lips 1-2cc. Marionette lines 2cc. Chin 1cc. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ I am not saying that every 40-year-old needs 16 syringes at their first filler visit, far from it. First of all, we don’t age in every part of our faces uniformly. So, while your upper face and lower face may look just fine, you may have noticed that you are getting lower lid hollowing which would take one to two syringes to correct. For those over 40, these numbers depend on the rate of aging and are most likely going to be higher. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ So do not fear the number of syringes, fear an unqualified injector. The Chin and Myomodulation. This lovely client wanted a very subtle chin enhancement. Here I used just one syringe (Juvederm Voluma 1ml) to treat her chin and activate the process of myomodulation by injecting under her chin muscle, mentalis. Note how myomodulation which involves activation of the elevator muscles (here by injecting mentalis) has helped reshape her whole face with a change in emotional appearance. Defining the angle of the jaw and tightening the jawline; part of Facial Reshaping. As we age, the bones of the face shrink and the soft tissues lose their support to produce a tired and ‘saggy’ appearance. Facial Reshaping with Myomodulation using just one syringe of filler (the client’s lips I treated in a previous session using my Nonsurgical Lip Lift, NLL). Here I used Juvederm Voluma (1ml) to treat her chin and the angles of her jaw. I knew that by injecting just a small amount of filler under her chin muscle, mentalis I would activate the elevator muscles of the face for a lifting and reshaping effect. This is the process of myomodulation and in my opinion it is the safest and most effective method of facial rejuvenation. Treatment of the lips starts with Facial Reshaping, here the cheeks and chin (top left) with the effects of myomodulation (bottom left video). I then used my Nonsurgical Lip Lift (NLL) which can involve shifting over the naturally occurring pillows of the lips called tubercles (tubercle shift shown in the video on the top right) with the final lip result (bottom right). These are the steps I took to achieve this lovely client’s final results (see also yesterday’s post). I used just two fillers (2mls) in total. When working with the natural anatomy and physiology of the face, then very little product is required to achieve subtle and beautiful natural looking results.
