Asteroid hyalosis is a degenerative condition of the eye involving small white opacities in the vitreous humor. Clinically, these opacities are quite refractile, giving the appearance of stars (or asteroids) shining in the night sky—except that ocular asteroids are often quite mobile. Ocular asteroids must be distinguished from the more common typical vitreous floaters, which are usually fibrillar or cellular condensates. The cause of asteroid hyalosis is unknown, but it has been associated with diabetes mellitus, hypertension, hypercholesterolemia, and, in certain animals, tumors of the ciliary body. In dogs, asteroid hyalosis is considered to be an age related change. The asteroid bodies are made up of hydroxylapatite, which in turn consists of calcium and phosphates or phospholipids. While asteroid hyalosis does not usually severely affect vision, the floating opacities can be quite annoying, and may interfere significantly with visualization and testing of the retina. While treatment of asteroid hyalosis is usually unnecessary, vitrectomy may occasionally be indicated, for both diagnostic and therapeutic purposes. It is an important test in the evaluation of patients at risk from glaucoma. Most tonometers are calibrated to measure pressure in millimeters of mercury (mmHg). Rebound tonometry. Rebound tonometers determine intraocular pressure by bouncing a small plastic tipped metal probe against the cornea. The device uses an induction coil to magnetise the probe and fire it against the cornea. As the probe bounces against the cornea and back into the device, it creates an induction current from which the intraocular pressure is calculated. The device is simple and easy to use and self-use versions are available. Eyelid Nevus An eyelid nevus can be congenital (since birth) or acquired. The acquired nevus usually appears between the ages of five and 15 years, and remains relatively quiet for the remainder of the patient’s life. The eyelid nevus is painless and can be pigmented or nonpigmented, flat or elevated. Sometimes, there is a minimal (less than one percent) risk for nevus to grow into malignant melanoma so these lesions should be evaluated by an eye doctor. Congenital dacryocele. Congenital dacryocele is an uncommon congenital anomaly characterized by bluish gray cystic swelling of lacrimal sac seen just below the medial canthal tendon. It is also referred to as congenital dacryocystocele, amniotocele, and mucocele. The condition is usually unilateral and has a female preponderance. It is typically present at birth and occurs due to concomitant obstruction of lacrimal system below the lacrimal sac and at the level of canalicular opening in the sac. There occurs entrapment of fluid nature of which is debatable. Some reports point to the source as amniotic fluid; however, some studies propose it to be mucus produced by intraluminal goblet cells. Diagnosis can be made on the basis of time of onset, appearance, ultrasound, computed tomography, or magnetic resonance imaging to differentiate it from meningoencephalocele, capillary hemangioma, and dermoid cyst. The swelling resolves spontaneously in majority of cases with conservative management, but when self-resolution fails to occur, early probing is warranted to prevent risk of dacryocystitis.
