A great little video showing the start of an ECCE (Extra-Capsular Cataract Extraction) courtesy of my great colleague and mentor @londonretinasurgeon . . . Due to the amazing advances in phaco technology, this is something we rarely see in the developed world practice, nevertheless get to perform! So sadly it is a skill being lost quickly…BUT it is still sometime the surgery of choice when you just KNOW that phaco will inevitably lead to failure. In this case for example, we have a very dense brunescent cataract with phacodenesis on slit lamp examination. In this circumstance, a planned ECCE is much safer and less traumatic to the eye than a failed phaco/dropped lens/PPV/Frag! . . . The surgery starts with a normal CCC. After a complete CCC of good size, several ‘relieving incisions’ are made in the round CCC to allow for expulsion of the whole lens from the capsule. . . . A large 2 step incision is then made (in this case using corneal scissors). . . . The lens is then generously hydrodissected, and ‘delivered’ using pressure inferiorly using a squint hook, and pressure behind the wound edge to open the wound. . . . A rough continuous/overlapping butterfly suture is applied to the wound quickly to reduce the chance of an expulsive haemorrhage, and the cortical material removed using SimCo . . . Колориметрическая классификация Л. Буратто, где градация заболевания производится с учетом изображений, получаемых при исследовании патологического хрусталика в свете щелевой лампы. Согласно ей, принято различать 5 степеней плотности ядра мутного хрусталика: Первая степень (1) - мягкое ядро, прозрачное или светло-серого оттенка, с кортикальными или субкапсулярными помутнениями, появившимися, как правило, недавно. Такое ядро особенно часто встречается в случае пресинильных метаболических катаракт. Вторая степень (2) - ядро малой плотности, ядерная катаракта серого с желтым или светло-серого оттенка. При этом, если ядро прозрачно, то оно имеет малой плотности консистенцию. Третья степень (3) - ядро средней плотности. Такое ядро характерно для классической возрастной катаракты. И если катаракта ядерная, цвет его будет желтым. Серый цвет ядра характерен для катаракт с кортикокапсулярным компонентом, которые встречаются у пациентов после 60-65 лет Четвертая степень (4) - плотное ядро янтарно-жёлтого цвета, встречающееся в случае перезрелых возрастных катаракт со значительным увеличением ядерной части, что говорить о длительно продолжающемся процессе помутнения. Пятая степень (5) - очень плотное ядро, имеющее темный цвет, оттенки которого могут изменятся от янтарного и до чёрного. При этом ядро занимает практически весь объем хрусталика.
