Today I used my cane outside of my apartment. We walked Lois in an area with less people, while Kirk occasionally sang Willy Wonka songs to make me smile about the cane. Ya know the song? “Come with me, and you’ll be, in a world of pure imagination...”. I’m shocked what a huge help it was. I didn’t make eye contact with a single person. I have a really hard time accepting this is where we’re at in this journey. But regardless, I’m really really really really proud of myself. Lk at this super cool video of a real-time MRI that shows a side view cross-section of the the entire trunk during respiration (fancy word for breathing). One of the very first things I discuss with patients when treating ANY type of pelvic floor dysfunction is breathing mechanics, and the reason for this is because we are meant to move—even our viscera (fancy word for organs): Our lungs expand 20,000x per day w/ breathing. Our livers (which weighs up to 3lbs) moves up and down with each breath. Our kidneys drop up to 3cm down with each full diaphragmatic breath. The uterus drops down 3cm into the pelvis before menstruation. . All of our organs are expected to move/glide with every-day functions like breathing and digestion. They’re attached to the frame of our bodies through layers of connective tissue and ligaments, and the ones that need arguably the most capability of movement live in the pelvis; The bladder, rectum and uterus need to be able to expand in order to allow proper filling and emptying. . As much as I love treating, training, and talking about the pelvic floor muscles, asking the pelvic floor muscles to squeeze (aka Kegel) and relax is only one piece of the puzzle. The pelvic floor is at the bottom of the pelvis, and it’s at the mercy of all of the pressure generated within and around the trunk. If the abdominal wall and ribcage are unable able to expand, the entire system works inefficiently which can affect pelvic floor function. . There are many reasons why our organs may lose their ability to expand or slide/glide: scar tissue from abdominal surgery, inflammatory diseases of the gut, chronic constipation, etc. But one way to encourage healthy visceral movement this is to BREATHE… breathe FULLY & DEEPLY. TRY IT: Lie on your back with one hand on your belly and take a full 5 seconds to slowly inhale. What did you feel? Did you feel your belly rise into your hand, or did it stay still? Diastasis Recti Considerations .. . . “What should I avoid with a DR?” Or “Is THAT exercise DR safe?” . . These are valid questions and I’m hoping to do a better job answering them. Moms are looking for a list of safe exercises which is awesome! But I really want to see you do it before I say it’s “SAFE” for you... . . I’m trying to be more clear as I coach moms through progression of movement. I want you to understand the concept more than a list.. . . Can you perform “that” exercise without straining/arching your back? Can you do that “movement” without doming in your abdomen? Are you holding your breath and putting excess pressure into your pelvic floor?.. . . Demonstrating how I check myself to see if I can do a a Pilates 100? A Hollow Hold? I’m working through a progression, learning to control my abdominal pressure, challenging my core, and stopping where my form fails.. . . Do you know how to do that? Here’s a great post by @dr.caleb.burgess showing healing timelines for our body’s tissues. . He makes a great point about many ways we can OPTIMIZE healing and it’s something I go over with EVERY. SINGLE. PATIENT! . Even if you can’t physically do everything you want to as we progress through rehab, making sure you’re taking advantage of what you CAN do will always be important ... ... ... Healing Times - One of the most common issues I encounter when helping people recover from an injury or from surgery is of the healing times of various tissues - Everyone wants to be better NOW, but this isn't realistic. Tissue healing takes TIME. We can't rush it. And it's important to understand that tissues heal at different rates depending on their physical makeup and blood supply, among other things. Additional contributing factors, such as diabetes, age, poor nutrition, inadequate rest, etc can further delay the healing process - This chart shows the AVERAGE range of healing times for various tissues in the body, including different grades of tissue injury for muscles, tendons, and ligaments. It can serve not only as a guide but also a reminder that we can't force our bodies to heal faster than physiologically possible - What we can do is OPTIMIZE the healing process by rehabbing appropriately, eating high quality food, getting adequate rest, and staying PATIENT and POSITIVE throughout the journey - ❤️ SAVE this chart as a reference and TAG a friend recovering from injury who could use it too!
