#Repost @lenadunham congrats on one year!! Keep coming back! ・・・ Today I’m in the miraculous position of being one year sober. I’ve done a lot of cool things in this life, but none has brought me the peace, joy and lasting connections that being part of a sober fellowship has (not even all girls camp. Sorry, Bunk Kingfisher.) Life is full of problems, but the cool thing about this one is that there is a solution: in every city, in many countries, you can find a group of people who are working hard to live sober, accountable lives and want to support you on your quest to do the same. I didn’t know I had an issue with drugs for a long time: because they were doctor prescribed, because I was outwardly successful and not a wild in da club party chick. But wouldn’t you say that hurting people you love is an issue? Wouldn’t you say feeling lost and lonely much of the time is an issue? Wouldn’t you say wearing shorts to a movie premiere *is* an issue? Sobriety hasn’t fixed my world. Life is still challenging- that’s the nature of the game. But every day I am surprised by the richness and depth of, well, reality. I don’t need to escape this beautiful carnival. Instead, I’m on the ride. Please remember you are never too far gone, too broken or too unique. There are people in plain sight waiting to help you. Let’s do this.

The Best Stretch That We Know Tight Back? This is by far one of the most beastly and effective ways to stretch the Quadratus Lumborum (QL) in the lower back. This QL stretch can be held for anywhere between 10 seconds to 2 minutes per side. You will get a stretch in numerous places, but the goal here is to feel a stretch deep in the side of the low back. . . Depending on the orientation of your hips and the position of your arms/legs, you may feel a stretch in these muscles as well: latismus dorsi, intercostal muscles between the ribs, external and internal obliques, tensor fascia latae/ileotibial band on the side of the hip/thigh, glutes, and peroneals on the side of the leg. . . Just about everyone can do this stretch to some degree. If you are very tight, don’t worry about getting as low as I do in the video. We have had hundreds of people, including myself, do this stretch with a disc injury and it definitely helps to relieve disc and sciatica symptoms. This QL stretch can also help to relieve joint (SI and Facet) and muscle pain in the low back. . . Like we always say, stretches are great and all, but to address the root cause of your pain and tightness, you must relearn your movement patterns. WORKING WITH PERSISTENT NERVE PAIN . Many of the patients I work with have dealt with persistent pain and limitations for a long time. And many of those happen to have persistent nerve pain. . Nerve pain can freak us out. Acute bouts of it will likely calm down in time, as most things do, if you back off from aggravating activities and try to remain active within tolerance. . But for others, the sensitivity decreases but never really leaves. It sticks around and feels like it is always there with certain movements or flares up after large bouts of activity. And to be honest, I don't always know exactly why this happens or why some people deal with symptoms longer than others. . What I do find very useful for them though is looking forward rather than back and finding small goals we can move towards hitting. . When I'm working with someone with persistent nerve pain, one thing we do is to gradually work on getting them to tolerate being in a more tensioned position for that nerve. . Now, the nerve has the ability to lengthen. It can get there, but sensitivity often limits it, often not even due to any sort of structural cause. And this can carryover into day to day activities amongst other things. . So if the symptom trigger is them going into that more tensed position, we will gradually ease our way there so that it is less sensitive and not as bothersome. . Usually we work for about 30 seconds at a time, seeing how they handle it and then walking around or resting in between. Some people stay there and others push up to around 60 second sets. But if there is any increase in symptoms that doesn't settle quickly, we will work at a less intense position or for less time. . We typically couple this with focus on a couple meaningful activities. Sometimes that is walking or hitting the gym or playing a sport or time spent playing with their kids. But we try to find what they can do and increase the dosage as tolerated. . Moving beyond persistent pain isn't always easy. It takes time. It takes work. And it takes keeping your eye on not only the prize, but also the process. Set small goals and cherish small wins. . You can get there. One day at a time. . #Prehab101
