Today marks one year of being a nomad with this one. Couldn’t have imagined how much the experience would challenge, strengthen and excite us. I can’t decide if it’s flown or crept by, but as I look back on the mileage we’ve covered in the past year - literally and figuratively - that I realize how much the whole experience has enriched our lives. One thing that’s changed in the past year is that we have no idea when we’re going to stop or where we’ll land, and we’re good with that. And one thing that’s stayed the same is our reliance on our creator for guidance throughout the process and for continually giving us a hope and purpose for our lives that’s greater than any city or road trip can offer. We’re just thankful. Andrea, I’d follow you anywhere. Almost. Congrats on 1 year as a #travelnurse!

Always CYA (cover your a$$) ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ But more importantly☝, documenting is about trending and understanding your patient. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ The only way that you’re creating an accurate picture about what is happening with your patient, among other things, is by documenting the events around your interventions. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ I am totally guilty of getting so caught up in my intervening that I never justify via my documentation why it was completed. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ You know you’ve done it correctly when another person reviews a chart without you and can understand exactly what happened. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ You know when the physicians round and they want to understand why you gave 20 PRNS overnight(total exaggerration) ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ You aren’t able to advocate for what you can’t prove! Pain medicine didn’t work? Well how will they know if you haven’t mentioned pre & post pain levels‍♀️ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ EXAMPLES. Administering Zofran? Pre-Documentation signifies pt c/o nausea or had an actual emesis Post-Documentation shows they have reported relief and they are now able to tolerate PO ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ Same with Pain, same with sedation. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ If you are administering sedation of any kind, Show me why it was necessary. Complete an agitation/behaviorial scale for your specialty area both pre & post intervention. Do you ever have those days where you just end up flat on your back? Not like looking at the stars or relaxing at the pool.
