Anatomy books show everything as neat separate muscles, bones, organs, ligaments, etc. But all of those things are actually bound together by fascia! In fact in our anatomy classes in PT school we were instructed to cut away or “clean up” the fascia so we could see each structure as separate. ✂️ ~ The pelvis is a unique crossroads for so many fascial attachments! As you can see in the video, your pelvis is linked all the way up to the head, and though you can’t see it—all the way down to the toes. ~ Long story short, if your pelvic PT isn’t assessing and treating at all the other parts of your body, they may be missing a key reason why you have pelvic pain or symptoms. Diastasis recti is the separation and thinning of the connective tissue that connects (get it? connective tissue...connects, haha!) the two sides of the abdominal wall. This creates less tension around the entire core (from the shoulders to the hips and the front to the back) and can cause problems in other areas of the bod. ... There *are* a few things you can do for cases of diastasis recti that are less than 2 fingers wide (if it's larger than that, find a physical therapist that specializes in healing diastasis recti). Some of them include learning to breathe properly and working your transverse abdominals with anti-rotation exercises. Things NOT to do: sit-ups, crunches, planks, and even bracing your core like-you-used-to while lifting weights. ... How does the Pelvic Floor relate to breathing? ⠀ The Pelvic Floor muscles are functionally related to the respiratory (thoracic) diaphragm. The relationship between the Pelvic Floor and the respiratory diaphragm goes further — they are both major players in breathing. While we often think of the respiratory diaphragm as the breathing muscle, there are also accessory breathing muscles. ⠀ The PF muscles actually form their own diaphragm — the pelvic diaphragm! As shown on this illustration, when you take a breath in, your respiratory diaphragm expands and descends. This movement of the respiratory diaphragm pushes all the organs beneath it in the pelvic cavity downwards. The pelvic diaphragm responds by expanding and descending to receive the organs. ⠀ During your exhale, your Pelvic Floor naturally contracts and lifts. In fact, the Pelvic Floor co-contracts with the transverse abdominis — the deep abdominal muscle we looked at above. Baby door muscles Yes friends! Credit for this concept goes to my dad, an ever faithful myPFM ambassador that never met a stranger. And then as I was trying it out, it worked out so perfectly to have the baby exiting the canal. I hope you love it too♥️⁣⁣ ⁣⁣ So, am I really advocating that every birth mother receives a pelvic therapy evaluation? Unequivocally, YES!⁣⁣ ⁣⁣ Why you ask? ⁣⁣ In simple terms, the PFM go through incredible changes during pregnancy and vaginal delivery (equal to a major sports injury per Dr. Bo, a world renowned pelvic physio that has conducted hundreds of research projects). ⁣ Many women may not know they have PFM dysfunction. ⁣⁣ Pelvic therapy helps to identify issues and rehab accordingly. We also help to educate on correct activation patterns and reinforce prevention strategies.⁣⁣ ⁣⁣ Research info:⁣⁣ “Vaginal delivery may lead to structural and denervation changes in the PFM, but also to secondary changes in their activation patterns.” Bo et al, p. 41⁣⁣ Up to 30% of healthy women cannot readily squeeze their PFM correctly on command. Bo et al, p. 40 (some research suggests a higher %)⁣⁣ 25% of women suffer from one or more pelvic floor dysfunction. Dieter 2015⁣⁣ About 50% of women have pelvic pain during pregnancy and 25% after pregnancy. B E S T B I R T H P O S I T I O N S F O R P E L V I C F L O O R This is something I teach all our Bump Method prenatal students. If possible, try to avoid delivering on your back in lithotomy (legs in stirrups) position. This position might be convenient for obstetricians and other assistants to observe what is happening during a birth, but it isn’t an instinctive birthing position. In fact it’s probably the worst way to try and give birth. In this position you are working against gravity and expecting your baby to go ‘uphill’ before she can get out! It also narrows the pelvic floor opening and prevents the sacrum from moving freely. Studies show there is more trauma to the pelvic floor when pushing in lithotomy position. There can also be increased risk of nerve compression and damage if in the lithotomy position for extended periods of time.. (Scroll for a video where I explain more and show images). ✨ Because of my heart condition I was at a level 3 hospital with an OB - midwives were not an option for my “higher risk” birth. At my hospital, OB’s won’t deliver babies if mom is on hands and knees or squatting (although hands and knees felt most intuitive for me at the time!) When it was time to push and I was asked to put legs in stirrups, I requested (more like Tarzan yelled ) to be side lying with one foot on a squat bar. This took pressure off my sacrum and allowed my pelvic floor to be more open. Baby G arrived safely in a few pushes, and mama’s pelvic floor was ok This is a simple tip that I hope is useful for pregnant mamas everywhere. Finally, try not to hold your breath when pushing. Keep exhaling through an open mouth. This will help reduce the downward pressure on your pelvic organs and minimize prolapse. Find out why so many surgeons are overlooking and missing endometriosis during laparoscopy.⁣⠀ ⁣⠀ This post shows a couple of surgical photos that demonstrate one of the more common errors surgeons make in performing a diagnostic laparoscopy. ⁣⠀ ⁣⠀ If the laparoscope is far away from the tissue it provides a panoramic view, but the size of the object is very small, even smaller than real life. Kind of like holding a book really far away and while you could see the whole book you may not be able to see the individual letters on the page. ⁣⠀ ⁣⠀ As the laparoscope was moved closer to the tissue a smaller area of tissue is visualized but the image is magnified. When the laparoscope is almost touching the tissue it is known as near contact laparoscopy and provides that maximum magnification of about 10X.⁣⠀ The first picture was taken far away from the tissue and allows us to see most of the uterus and bladder and there is no #endometriosis seen. ⁣⠀ ⁣⠀ The second picture is much closer and we see a smaller area of the uterus and bladder now we can see multiple areas of endometriosis. We go back and look at the first picture we can see that even though we know endometriosis is present, it cannot be seen. ⁣⠀ ⁣⠀ Without performing near contact laparoscopy is very easy to miss obvious endometriosis and thus leaving the patient with pain causing lesions.⁣⠀ ⁣Make sure your surgeon understands the importance of near contact laparoscopy before having endometriosis surgery.
