Out of the boot? Now what? —— Some people are in a boot for a lengthy amount of time which can cause proprioceptive issues and muscle atrophy. Once they’re out of the boot, my goals are to improve both through different exercises. I think this is a mismanaged area of orthopedics as many people may never end up doing rehab after lengthy bouts in a boot. In my opinion, this puts them at risk for another injury by returning to sports too soon. —— Here are a few exercises we started working on right when she got out of the boot. She was still rehabbing while in the boot in order to combat muscle atrophy. 1️⃣ Balance board - working on proprioception is a must. Being in a boot for lengthy amounts of time can affect proprioception. 2️⃣ Tandem balance with kettle bell swing 3️⃣ Calf raises - muscle atrophy is important to address and the calves can’t be neglected. 4️⃣ Deadifts - pick up heavy weights right? —— There needs to be formal education on ACL rehab which is why I started an ACL Mastermind Group. Quad Strain Rehab ——- When addressing a muscle strain, gradually increasing the dosage to the muscle is the key. The muscle needs to “rebuild” and regain its ability to handle load. I want to challenge the muscle in as many ways as possible to ensure it can handle all the demands of sports. In regards to the quad, hip extension with knee flexion under load is one of the more challenging positions. Many people restrain a quad when trying to kick a ball so increasing the tolerance in that position is very important for full recovery. ——- Here are a few exercises I use in early stages of rehab. The goal is to challenge the muscle without pain. 1️⃣ Eccentric step down with dowel - working on eccentric quad control with a hand held assist to take some pressure off and so she doesn’t need to worry about balance. 2️⃣ Eccentric step down - progress with no dowel. 3️⃣ TRX pistol box squat - same concept as the last exercise, focusing on eccentric with some assistance. 4️⃣ Pistol box squat - again focusing on eccentrics only as concentric typically will be more irritating to the muscle in the acute phase. ——- There needs to be formal education on ACL rehab which is why I started an ACL Mastermind Group. Petite « update genou » après pratiquement deux semaines passées sur mon canapé qui m’ont parues une éternité... Ce matin j’ai eu mon premier rendez-vous post opératoire avec mon chirurgien où j’ai enfin pu voir l’état du genou et des cicatrices. À ma grande surprise ce n’était pas si terrible que cela et je suis même plutôt satisfaite du résultat!! J’ai décidé de faire l’opération aux Etat Unis car j’ai pu avoir accès à une technique qui ne se pratique pas encore en France ou que très rarement. La plupart des gens qui se font opérer du ligament croisé antérieur utilise leur propre tendon pour la reconstruction. Il se trouve qu’ici j’ai eu l’option de faire appel à un « donneur » pour la greffe d’un nouveau ligament. La cicatrice est donc plus esthétique ( bcp plus petite), la douleur post chirurgie plus tolérable et la rééducation sensée être un peu plus « simple ». Le chemin est encore bien long mais petit à petit ça avance. Bonne nouvelle de la journée, je ne dois plus porter d’attelle. Je ne la supportais pas du tout donc vous m’en voyez ravie. Je dois encore utiliser les béquilles pendant une semaine environ et dès que je m’en sens le courage je pourrai m’en débarrasser, ce qui n’est pas facile du tout n’ayant pas mis de poids sur ma jambe gauche pendant plus de deux semaines. Le plus difficile comme dirait mon médecin est que mon esprit fasse confiance à mon corps ce qui pour le moment est assez compliqué, beaucoup d’appréhension. Déjà marcher sans attelle est un sentiment très bizarre. La route est encore bien longue, je n’arrive pas du tout à tendre ma jambe, il faut que je réapprenne à marcher mais je commence la kiné dans les prochains jours et ce 3 fois par semaine pendant 3 mois donc préparez vous à pas mal de vidéos physiothérapie ! C’était le point médical du jour oh et j’oubliai presque, un grand merci à mon chirurgien @dr_sportsmedicine De @kerlanjobe pour son efficacité et sa disponibilité!

Single Leg Balance Variation ——- I utilize single leg balance exercises to work on neuromuscular control and improve proprioceptive awareness in lower extremity patients. I like to utilize various versions of a seemingly simple task. This not only offers various challenges but it also keeps them engaged. ——- Here is one variation I like to use when addressing single leg neuromuscular control. We’re using a med ball and dropping into a single leg RDL. She is then bouncing the ball three times before raising the ball above her head. ——- There needs to be formal education on ACL rehab which is why I started an ACL Mastermind Group. ACL Rehab - Plyometrics —— Jumping and landing are a huge part of sports. As soon as they’re physically ready, I incorporate jumping and landing exercises in rehab. Plyos are important to work on neuromuscular control and is also a way of challenging muscle strength. A few things I look for include: 1️⃣ Do they shift while loading up to jump 2️⃣ Do they shift when they land 3️⃣ Does one foot take off or land before the other 4️⃣ Are they landing with any compensations such as knee valgus, hip rotation, trunk movement 5️⃣ Do they have to look down during jumping/landing 6️⃣ Subjectively, does it feel the same as the other leg? —— Utilizing different types of plyometric exercises allows for the athlete to be adequately challenged and can also help reveal potential compensations. Here are some examples of plyometric exercises of different difficulty levels I like to use: 1️⃣ Banded box step offs - the band is pulling her down requiring increasing demands on her quads to decelerate and push back to the start position. 2️⃣ Lateral hop to bound - working on neuromuscular control as I want her to stick the landing. 3️⃣ Same as above but to the opposite side. Make sure you work on both sides! —— When can you do these? It depends! —— There needs to be formal education on ACL rehab which is why I started an ACL Mastermind Group.
