This is my Lexapro. It is an anti-anxiety medication that significantly helps me manage the day to day mental burden of my diabetes. I post this because I am NOT ASHAMED of the steps that I take to stay mentally healthy so that I can stay physically healthy. Though this drug by itself does not help control my BGs, it is almost as important to me as my insulin.’ • I have struggled my whole life with anxiety. Don’t get me wrong, a certain level of anxiety in life can be healthy, but my anxiety, when uncontrolled, tends to both damage my relationships, and inhibit my ability to maintain good BG control. • Studies have shown that diabetes and anxiety go hand in hand, likely because practices necessary for good BG control are also anxiety promoting. I am constantly pondering the “what-if’s” when it comes to my DM… “what if I…give myself too much insulin, not enough, work out later, don’t finish my lunch.” Or the scariest question of all, “what if I’m low overnight and I don’t wake up?”. Though planning is essential in DM management, it can often lead to irrational, “what-if’s” that are detrimental to control. • The first step in managing anxiety with diabetes is identifying it and asking, is this rational or irrational anxiety? The fear of nighttime hypoglycemia is rational, but running my BG up to 250mg/dl before bed because of that fear, is not. Not only do I have a CGM, I have a hybrid-closed loop pump that will prevent me from even getting close to that low. For me, this is an irrational fear. • Unfortunately, even when I identify anxiety as irrational it still prevents me from living in the present. Additionally, my anxiety and BGs are directly correlated. High anxiety = high BGs. It’s a vicious cycle! This is why I decided to start on my medication. I’m not saying that everyone with diabetes and anxiety needs to be on anti-anxiety medication, but this is what works for me and I’m much happier because of it. My relationships have also improved because I’m not on edge all of the time. • What do you guys do to relieve your DM anxiety? • This post is a bit personal, so positive comments only please

Whether you’re using long-acting basal insulin or insulin pump therapy, an accurate basal dose of insulin is imperative to provide a stable baseline for carb and correction boluses. It allows for steady fasting BG levels, minimal BG fluctuations, and more freedom regarding meal timing (someone with an accurate basal should be able to skip meals without going low). Once an accurate dose is established, it is much easier to establish a carb factor (insulin to carb ratio) and correction factor (insulin to BG ratio). • So, how do we know if we are taking the right amount of basal insulin? We do basal testing! When taking an accurate basal dose, we should have a relatively flat glucose pattern when no food is eaten. Basal testing should be done when switching to a new basal insulin or when a basal dosage is changed. • Basal testing can be done in three different sections; overnight, first ½ of the day, and second ½ of the day. The overnight basal test is the easiest because events that cause hypo or hyperglycemia (exercise, stress, food) are usually stopped. Start your overnight basal test at least 4hrs after your previous rapid-acting insulin dose (due to the 4hr duration of action of rapid insulin). You should begin your basal testing at a relatively good BG (100-150mg/dl suggested). Do not eat or give another dose of rapid acting insulin before going to bed. Test BG at 2am and again upon waking (or every 2hrs if you wish). CGM’s are great for basal testing. With an accurate basal dose, your BG should not have risen more than 15mg/dl or fallen more than 30mg/dl overnight. If you go low during this test, stop and treat. If your overnight basal dose does not meet the described standards, raise or lower your dose (0.5-2.0U) and try again! • Continued...

Yesterday, I came across the super old physician reference guide, “Our Family Physician”. You guys…this was written in 1887 and was actually used to treat common disease in the late 1800’s! Of course, I immediately looked up diabetes and was both impressed and fascinated. I thought I’d share… • Diabetes as told by “Our Family Physician”: • “This is a disease in which large quantities of urine are passed, sometimes amounting to several gallons in twenty-four hours.” • “Symptoms. –The appetite is voracious, and there is great thirst; dry skin; the digestion is imperfect; the gums swollen and inflamed; dry, parched mouth; waste of flesh; debility, pain and weakness in the loins; loss of sexual appetite; frequent attacks of dizziness and pain in the head; the urine contains a large amount of sugar, for which there are several tests.” • “Causes. —It may be caused by any means that derange the digestive functions; intemperate habits; great fatigue; constant use of acid drinks; use of Mercury; exposure to cold.” • Obviously, this description is extreme considering the fact that insulin was not even invented until the 1920’s, but I still feel like it holds up pretty well! Especially to those with prolonged uncontrol or who are withholding insulin for one reason or another. • Here is an excerpt of the recommended treatment… • “The patient should not be allowed to eat articles which contain sugar or starch, such as potatoes, beets, parsnips, and other vegetables.” • Obviously, we’ve modified the diet a bit, but…IM IMPRESSED. • See the pictures above for all of the treatment recommendations.
